- Contact information — name, phone number, email address, and relationship to patient, submitted via our contact form or during a phone enquiry.
- Clinical information — substance-use history, withdrawal risk, co-occurring medical conditions, psychiatric history, and family context, collected during the admission assessment.
- Medical records — all records generated during residential treatment, outpatient follow-up, and aftercare. Stored securely and retained in accordance with applicable Indian medical-record retention requirements.
- Technical data — IP address, browser, device type, and pages visited. Used for rate limiting the contact form and aggregate analytics.
How we use your information
We use information for the following purposes only:
- To respond to your enquiry and arrange admission if clinically appropriate.
- To deliver medical treatment and coordinate care across the treatment phases.
- To communicate with primary family contacts during the patient’s treatment.
- To issue invoices, receipts, and written documentation that the patient or family request.
- To comply with applicable legal, regulatory, and accreditation requirements.
Confidentiality
Patient and family information is handled with strict confidentiality. We do not share patient information with employers, other family members outside the primary contact, insurance providers, or any third party without the patient’s explicit written consent — except where compelled by law (for example, court orders or mandatory reporting of specific clinical situations).
Data retention
Medical records are retained in accordance with applicable Indian medical-record requirements. Contact-form submissions are retained for 24 months for continuity of care before being archived. You may request deletion of your contact details at any time by emailing help@simranshri.in, subject to statutory retention obligations for medical records.
Cookies and analytics
Our website uses only essential cookies required for core functionality. We do not use cross-site tracking cookies, third-party advertising cookies, or behavioural-profiling tools.
Third-party services
Our contact form uses Resend for email delivery to our admissions inbox. Our website is hosted on Vercel. Our map display uses Google Maps. These services process technical data (IP addresses, request logs) as part of their standard operation. We do not transfer any clinical or medical information to these services.
Your rights
You have the right to:
- Request access to personal information we hold about you.
- Request correction of inaccurate information.
- Request deletion of non-clinical personal information (subject to legal retention obligations).
- Withdraw consent for specific non-essential uses at any time.
